Birth certificate corrections
A difference in a name, date or other entry requires a review of the record and the office that issued it. The appropriate route depends on the error and the supporting documents.
Information for the initial review
- Country, state and registry that issued the certificate.
- Which entry appears incorrect and what the supporting records show.
- Where the corrected certificate will be used.
- Any previous request or response from the registry.
Scope and quotation
The proposal should specify the work, expected deliverables and follow-up. Professional fees, official fees, translations and shipping are itemized separately when applicable. Requirements, appointments and final decisions depend on the relevant authority; processing times and results are not guaranteed.
Start with a short description. Avoid sending full identity documents or sensitive personal information in your first social-media message.
